Please use this form to help us prepare for your evaluation. This is not your typical evaluation — it’s also an introduction to what our private lessons are like with one of our trainers.
Who needs to be present? Anyone who will be involved in the dog’s day-to-day living should be at the evaluation. This allows us to see how the dog interacts with each family member.
Feeding notes: Please do not feed the dog an early meal — we want to see their food drive (how well they eat their dog food), and how motivated they are in accepting rewards. Please have some different treats available (cheese, meat bits, etc.).
Toys: Have all your dog’s toys available for your evaluator to browse through. This helps us evaluate and see their play drive (how gentle or rough the dog plays).

Please list any and all behavioral issues. Don’t be shy — we are here to help with all of them.
